Using AI-anabled remote monitoring to support patients with chronic disease

Chronic disease, limited access to primary care and health human resource shortages continue to place pressure on health systems. This session will explore findings from a Canadian primary care pilot that used AI-enabled remote patient monitoring, telehealth and predictive analytics to support patients with chronic conditions. 

Participants will learn how continuous biometric monitoring, clinician dashboards and automated alerts were integrated into primary care workflows to support early intervention, risk stratification and care continuity. The session will review implementation considerations, pilot outcomes and the potential role of remote monitoring in reducing unnecessary acute care utilization while supporting clinician efficiency and patient-centred care.
