
Simplifying Preventive Care for Older People with a Novel EMR Reminder Tool
In this session, learn about the free, point-of-care for preventive care for older people (PCOP) tool from the Centre for Effective Practice (CEP). By easily integrating with TELUS PS Suite Electronic Medical Records (EMRs) and facilitating the latest evidence into practice, it can help clinicians provide fulsome preventive care for aging patients. Health behaviours (physical/sedentary activity, social isolation, loneliness), fall prevention, cardiovascular health, cancer screening, fragility fractures, immunizations and more will be discussed.

Presenter Bios:

Amy Freedman MD
Dr. Amy Freedman is a family physician at St. Michael’s Academic Family Health Team in Toronto, Ontario. She practices broad spectrum family medicine as well as having a focused practice in care of the elderly. She is the Physician Lead for the SMHAFHT home visit team and practices in long term care and supportive housing at Baycrest. Dr. Freedman is an assistant professor in the Department of Family and Community Medicine at the University of Toronto.

Makayla Gomes
Makayla Gomes is the Centre for Effective Practice (CEP) Project Coordinator for the Knowledge Translation in Primary Care initiative and manages project logistics and team communications. She skillfully engages primary care providers, system stakeholders and persons with lived experience to inform project scope, objectives and content development and ensure CEP tools succeed in addressing the evidence-to-practice gaps in primary care. Prior to joining the CEP in 2022, Makayla worked as a Research Assistant at Western University. Her research background in knowledge translation has enabled her to meaningfully contribute to the development of primary care tools for heart failure, anxiety and depression and the COVID-19 Resource Centre. Makayla holds a Master of Health Information Science from Western University.
