David Kaplan, MD

Dr. David Kaplan is a distinguished physician leader currently serving as Chief of Clinical Quality at Health Quality Ontario. He is a family physician who practices comprehensive family medicine with the North York Family Health Team and at North York General Hospital. He is a graduate of the University of Toronto and completed graduate training at the Institute for Health Policy, Management and Evaluation at the Joint Centre for Bioethics. A bold innovator, Dr. Kaplan was awarded the inaugural Integrated Medical Education Award from the University of Toronto’s Faculty of Medicine and the Innovation Award from the Society of Graduates of the Institute of Health Policy, Management and Evaluation for his significant contribution to the fields of health policy and health management.

Erin Keely, MD

Dr. Erin Keely is an Endocrinologist at the Ottawa Hospital and Professor in the Department of Medicine at the University of Ottawa. She is Co-Founder of the Champlain BASE eConsult Service and is Executive Director of the eConsult Centre of Excellence. She is the Ontario Health Clinical Lead for the Central Intake Program which includes the development of standardized eReferral forms. Her research focuses on health care delivery and provider to provider written communication.
