Creating a Local Primary Care Digital Health Community of Practice 

Jeremy Chad, MD

Dr. Jeremy Chad is a family physician on the North York General Hospital Family Medicine Teaching Unit and one of the Digital Leads with the North York Toronto Health Partners (NYTHP) Primary Care Network. He has been a part of the NYTHP Primary Care Advisory Council since it was first formed in 2021. He has a wide interest in digital health technologies, including technologies that replace the fax system and artificial intelligence. He is the regional primary care champion for eReferral and he sits on the University of Toronto Department of Family and Community Medicine Digital Health Committee. He is a full-time Lecturer with the University of Toronto Temerty Faculty of Medicine Department of Family and Community Medicine.
