Short snapper – improving home care documentation with voice AI to support primary care
Home care observations play an important role in helping primary care teams manage frail seniors and patients with complex chronic conditions, yet documentation is often incomplete and inconsistent. This session examines how a voice-first documentation solution can create structured, SOAP-ready clinical notes that improve communication between home care providers and primary care teams while reducing documentation time. Using real-world implementation data, attendees will learn how voice-enabled documentation can strengthen continuity of care, improve the quality of clinical information and return valuable time to patient-facing care.

Presenter Bios:
Tayeb Toufik Dahar
Tayeb Toufik Dahar is the Director of Business Intelligence Innovations at Canadian Shield Health Care Services, with more than 20 years of experience in business intelligence, data strategy, and digital transformation. He led the development and implementation of PolyVoice Companion, a technology developed by Canadian Shield Health Care Services to support reporting, documentation, and operational management across primary care, home care, mental health, and other organizational functions. His work has included leading the development team, overseeing implementation, and directing the analytics used to measure adoption, documentation quality, performance, and operational results. Tayeb’s contribution to the OntarioMD Digital Health Conference draws on this work, including the use of voice-based documentation to improve the completeness of home care reporting, reduce administrative burden, and support continuity between home care and primary care. He also applies business intelligence and digital tools to areas such as performance management, health and safety, project reporting, and organizational decision-making.
Robin Cheslock
Robin Cheslock is the Clinical Director of Integrated Care Services and a Registered Social Worker with extensive experience across diverse clinical settings. His background spans crisis intervention, mental health assessment, psychotherapy, leadership coaching, organizational quality improvement, and staff wellness support.
