Explore how a province-wide digital perinatal record could transform the delivery of pregnancy care by reducing documentation burden and improving information sharing across care settings. This session introduces BORNConnect’s vision for an interoperable perinatal record that integrates directly within existing EMRs, supporting seamless transitions between primary care, obstetrics and hospital settings. Through clinician insights, emerging design concepts and interactive discussion, attendees will discover how digitally connected workflows can enhance continuity of care, improve access to critical pregnancy information and inform the future of perinatal care in Ontario.

Sophie Outar

Sophie Outar is Project Director at the Better Outcomes Registry & Network (BORN Ontario), where she supports initiatives aimed at improving maternal, newborn, and child health through better use of data and digital health. She brings more than two decades of experience in health system transformation and technology-enabled care in Ontario, including previous leadership roles at Ontario Health and within the province’s regional health system. Sophie has extensive experience in digital health strategy, program delivery, system integration, and bringing healthcare partners together to improve connected care. Previously, Sophie worked at Ontario Health implementing digital health and virtual care solutions within the East Region and working with a variety of partners and health organizations. She has a specialized interest in clinical and patient co-design and focuses on the use of data for evidence-based decision-making outcomes.

Sophie has a deep passion for improving the patient and caregiver experience having had multiple high-risk pregnancies. She has been involved in various projects and initiatives at Sinai Health System, Toronto providing her lived experience with the intention of improving the quality of care.
