Achieving Connected Care through Data: A Clinical Perspective

Join Dr. Chandi Chandrasena, OntarioMD’s Chief Medical Officer and Alyssa Bryan, RN, MScN, Canadian Institute for Health Information (CIHI) as they share their clinical insights and experiences in developing the health data standards to enable Connected Care and improve patient safety and ensure quality care.

These health data standards are bundled into the Pan-Canadian Health Data Content Framework that is foundational for Connected Care. It defines and standardizes the essential patient information needed in clinical care, resource management, population health and health system use.

Canada is now poised to advance its digital maturity and make interoperability a reality using a collaborative approach to inform, develop, and enable the work for data content and exchange standardization by including clinicians, jurisdictions, patient partners, Indigenous Peoples and vendors into the development process.

Presenter Bios:

Alyssa Bryan

Alyssa Bryan, RN, MScN

Alyssa Bryan RN, MScN is a registered nurse with experience working in critical care, public health and community health centres. Building on the clinical skills that Alyssa acquired in her 12 years of frontline nursing, she moved into standards development for healthcare accreditation. Within this role, her focus was on developing standards on primary healthcare, palliative care, mental health and addictions, health system integration, and cultural safety and humility. Since joining the Canadian Institute for Health Information, Alyssa has been focused on developing primary health care data content standards as part of CIHI’s pan-Canadian Health Data Content Framework. Alyssa is passionate about health equity, anti-racism, the social determinants of health and health data interoperability.

Chandi Chandrasena, MD CCFP FCFP

Dr. Chandrasena joined OntarioMD (OMD) as its Chief Medical Officer (CMO) in 2021. She is a family physician practicing in Ottawa for almost 20 years and a former OMD Physician Peer Leader who has mentored many of her colleagues to use EMRs and digital health tools effectively to enhance patient care. As CMO, she provides clinical advice at provincial and national tables and clinician advisory groups and is instrumental in forging system-level partnerships.
